Senior / Care Coordinator, Population Health & Integrated Care
You will coordinate care and case management for manage patients with complex care needs, maintaining comprehensive records of demographics, medical histories, and treatment plans while ensuring seamless care coordination and continuity. You will collaborate with multidisciplinary teams, including physicians and allied health professionals, to coordinate care plans, facilitate timely follow-ups, and manage referrals to community resources. You will also support population health research through data collection, collect treatment data and facilitate stakeholder meetings, and with various stakeholders. Additionally, you will organise population community health initiatives for the community and ensure all programmes comply with relevant regulations and standards , ensuring all works meet regulatory compliance.
Responsibilities- Coordinate care and case management for patients with complex care needs, maintaining comprehensive records of demographics, medical histories, and treatment plans to ensure seamless care coordination and continuity.
- Collaborate with multidisciplinary teams, including physicians and allied health professionals, to coordinate care plans, facilitate timely follow-ups, and manage referrals to community resources.
- Support population health research through data collection and data-driven decision making; collect treatment data and facilitate stakeholder meetings with various stakeholders.
- Organise population and community health initiatives and ensure all programmes comply with relevant regulations and standards, ensuring regulatory compliance across activities.
- Diploma with 8 years relevant experience or degree in a healthcare-related field
- Experience in care coordination, case management, or a similar role
- Knowledge of community resources and services
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
