You will serve as a navigator, educator, data analyst, and compassionate caregiver. This role offers the opportunity to directly impact patient care and contribute to efficient care coordination across diagnosis, treatment, rehabilitation, follow-up, survivorship, and end-of-life care.Job Responsibilities*
A. Direct Care1. Care NavigationProvide personalized informational and emotional support to patients and families, helping them overcome healthcare system barriers, ensuring continuity and quality of palliative care.
Identify high-risk patients and assist in navigating their care journey to prevent gaps in service and facilitate access to necessary resources.Follow up with patients via phone as needed to improve access to the medical team, reduce unnecessary emergency visits, and ensure ongoing care.Support patients and families in transitioning across different services and locations.Communicate effectively with healthcare professionals to ensure smooth care transitions.Assess patients physical, psychosocial, and spiritual needs, identify barriers, and initiate interventions to address these needs.Evaluate patient navigation outcomes.
2. Care CoordinationEnsure adherence to palliative care pathways and streamline both inpatient and outpatient care.Coordinate care transitions and appropriate patient placement, collaborating with community partners such as Palliative Home Care and inpatient hospice services.Facilitate timely referrals to specialists, allied health, and support services.Coordinate multidisciplinary care across services and settings, liaising with medical, surgical, and allied health professionals.Evaluate outcomes of care pathways.
B. Education1. Patient EducationEducate patients and families on palliative care approaches, symptom management, and available resources.Triage patients for subspecialty care.2. Staff ResourcesAdvise clinic staff on internal and community referrals, including financial counseling options.Support multidisciplinary team members regarding patient care matters.
C. Data ManagementCompile data, build databases, and conduct annual audits.Assist with data analytics and participate in quality improvement projects aimed at enhancing care coordination and outcomes.Collaborate with VDO teams to measure care outcomes.
D. ResearchCoordinate palliative care research and quality improvement projects.Liaise with research staff to ensure compliance with ethics and data governance policies.
E. Quality Improvement and National InitiativesWork with Singapore Hospice Council on national initiatives such as guidelines, data sets, surveys, and benchmarking.Participate in workgroups and pilot programs led by SHC and MOH.
F. Leadership and Professional DevelopmentFacilitate quality improvement activities.Support the expansion of palliative care services.Participate in strategic planning and other assigned projects.
