Raffles Medical Group is expanding our home care service, and we are looking for a doctor who is passionate about caring for patients beyond the clinic setting.
As a Resident Physician in Home Care, you will manage your own panel of homebound patients, including elderly individuals and patients with complex medical needs who may be unable to visit a clinic. You will follow them over time as their named physician, providing consistent and personalised care in the comfort of their homes.
Working closely with experienced community nurses and care partners, you will manage chronic conditions, conduct medication reviews, coordinate care plans and support patients and their families through different stages of their healthcare journey. This is longitudinal, relationship-based family medicine: the model GPs in the UK and Australia have practised for generations, now growing in Singapore's community care landscape.
What the role involves:
- Conducting home medical visits within a structured weekday roster
- Seeing approximately eight patients each day, with one-hour appointment slots and geographically clustered visits
- Managing a named patient panel to ensure continuity of care
- Working in partnership with an established community care provider
- Providing teleconsultations during office hours where required
- Collaborating with community nurses, caregivers and other healthcare professionals
- Managing chronic medical conditions, medications and ongoing care plans
- Providing guidance and support to patients’ families and caregivers
There are no night duties, weekend duties or on-call requirements.
You will also receive the full support of Raffles Medical Group’s primary care network, including clinical governance, peer support and guidance from the Medical Director’s Office. This is a full-time position with employee benefits.
What we are looking for:
- Full registration with the Singapore Medical Council
- Experience or a genuine interest in community care, geriatric medicine or palliative care
- GDFM, MCFM or FCFP qualifications are welcomed but not mandatory
- Confidence in practising independently outside a traditional clinic environment
- Sound clinical judgement and a warm, patient-centred bedside manner
- A commitment to continuity of care and building trusted relationships with patients and their families
