As a Community Care Associate for our Fun with Friends Programme, you are a dedicated care team that delivers a person-centred care model for our seniors, including persons with dementia. We aim to promote seniors independence and maintain their cognitive and physical functions through multi-modality group programmes, ensuring they lead fulfilling lives.

You will need to:

• Facilitate various multi-modality group activities for persons with dementia as assigned by the programme executive; including exercise sessions, meal preparation groups, cognitive games, music and art-related activities.
• Assist in activity planning and preparation to ensure the smooth execution of the group activities
• Manage clients with different mental, and physical conditions and behaviour of concerns relating to dementia and assist in the monitoring of clients health status
• Assist in clients care including providing necessary assistance for transfers, toileting and functional mobility
• Assist in administering assessments to clients and caregivers, perform data collection
• Perform timely documentation for the client post-session
• Assist to provide timely updates to family/NOK of clients engagement
• Support staff or volunteers relating to assist with running the programme
• Assist in inducting new community care associates and provide necessary guidance
• Assist in the procurement of activities materials, with approval sought from RO/Programme Executive
• Perform general housekeeping to the programme premise

• Nitec / Diploma / Certificate in Healthcare or equivalent course
• Some experience in social service or the healthcare industry would be an advantage
• Facilitation skills and healthcare knowledge would be an advantage

You should:

• Be comfortable in facilitating group sessions and programme
• Be empathetic and form positive relationships with persons with dementia and caregivers
• Thrive in a collaborative and dynamic environment
• Be proficient in English and Mandarin. The ability to converse in dialects will be an added advantage.
• Be proficient in Google Suite Applications/MS Office

Working hours:

• Five-day work week 8-5.30pm. Timing may vary according to programme needs
• Region-wide- West, North and East region, but might be required to move around for role
