Job Summary

Lead and supervise pharmacy teams to deliver advanced technical support, medication safety, and operational excellence. Provide expert counselling and training in pharmacotherapy, focusing on chronic disease management and improving patient quality of life.

Responsibilities

  • Supervise and coach pharmacy teams to coordinate medication supply workflows ensuring timely, accurate, and legally compliant dispensing
  • Investigate and manage medication supply errors or losses, implementing corrective measures to prevent recurrence
  • Guide teams to perform medication reconciliation services by resolving discrepancies with prescribers and documenting outcomes accurately
  • Anticipate medication safety and supply issues, developing and executing action plans with clinical teams
  • Mentor teams to assess and resolve medication-related issues through review, reconciliation, and optimization of pharmaceutical care plans
  • Provide medication-related counselling, training, and resource support to patients, public, and healthcare staff
  • Recommend evidence-based, cost-effective medication regimens and appropriate monitoring to healthcare professionals
  • Lead discharge planning by guiding teams to conduct self-administration programs and pill-box packing to improve post-discharge medication compliance
  • Ensure patients and caregivers receive counseling and resources aligned with standards of care and independently resolve post-discharge medication issues
  • Oversee inventory management including receipt, storage, monitoring, destruction, cycle counts, and investigation of discrepancies in compliance with regulations
  • Manage end-to-end procurement processes including sourcing, price negotiation, contract administration, and supplier relationship management
  • Monitor contract compliance, expiry, renewals, and variations while analyzing consumption trends and pricing benchmarks
  • Provide input for medication budgeting, cost containment, and waste minimization strategies
  • Mentor and coach pharmacy staff, serving as a pharmaceutical resource expert and contributing to multidisciplinary research and care planning
  • Assist in developing competency assessments and training curricula for clinical teams
  • Support quality assurance by identifying, developing, and auditing programs, and analyzing data to evaluate outcomes
  • Facilitate clinical information system management by supporting system enhancements, training staff, resolving issues, and conducting user acceptance testing
  • Manage medication-related databases including creation, updates, maintenance, and data analysis to support clinical decisions
  • Promote risk awareness among staff and support hospital-wide risk management efforts related to pharmacy operations

Required competencies and certifications

  • Degree in Pharmacy
  • Registration with the Singapore Pharmacist Board with a valid patient care practicing license

Preferred competencies and qualifications

  • Minimum 5 years of proven experience as a pharmacist performing the described responsibilities

Workdays

  • Monday: 8.30am to 5.15pm (includes one hour lunch break)
  • Tuesday to Friday: 8.30am to 5.30pm (includes one hour lunch break)
  • Saturday: Once every 4 weeks, 8.30am to 12.30pm
  • Total 40.75 hours per week
